Part of the application process includes references: three regular references and one spiritual reference. Please
sign the waiver section and give the forms to the individuals who will be serving as your references. You should
include a variety of people, both men and women, who presently know you well (i.e. employer, professor, peer, roommate).
Please note that the spiritual reference should be completed by someone who knows you in the context of your faith life.
Each reference should mail the completed form directly to our office in a sealed envelope with his/her signature across the seal.
