Mr. Formolo joined CHS Capital in 1990 and was promoted to Partner in 1997. Prior to joining CHS, Mr. Formolo was employed by Heller Financial, Inc. as a
Vice President in the Leveraged Funding Group. He holds a B.B.A. from the University of Wisconsin and an M.B.A. from Northwestern University. Mr. Formolo
serves on the Board of Directors of American Reprographics Company (NYSE: ARP), AMF Bowling Worldwide, Inc., Heartland Dental Care, Inc., QubicaAMF Worldwide,
S.a.r.L., Royall & Company, and WASH Multifamily Laundry Systems, LLC. Mr. Formolo is also on the Board of Trustees of North Shore Country Day School (formerly Board Chair),
and the Board of Directors of New Schools for Chicago (formerly Renaissance Schools Fund) and Chicago Fellowship. <br>
